If you or a loved one has been diagnosed with a chronic disease such as heart disease, high blood pressure, high cholesterol, stroke, lupus, arthritis, diabetes, COPD, Crohn’s or ulcerative colitis, a Chronic Disease Self-Management Workshop from M Power can help you take control of your condition, offering resources, support and health education at no-cost to you via your local county health department. Participants meet for two hours once a week, for six weeks

Developed by Stanford University, this free program is designed to assist you in developing a self-management program to help you stay active and enjoy doing the things that you love. It is for caregivers or people living with arthritis, asthma, lupus, fibromyalgia, heart disease, diabetes, COPD, emphysema and other chronic conditions. Topics include managing symptoms, stress and fatigue; nutrition and exercise; dealing with difficult emotions; effective problem solving; forming a partnership with your health care team and more.
